Congratulations to Michelle H. for receiving the 2025 New Hampshire Healthcare Hero Award! Last week, Michelle, supported by her center team and GHC staff, was recognized for her dedication to caring for the geriatric population. Here's a quote from Michelle's manager, Julianne, detailing the service and extra care Michelle has provided while working with AlignMed: "Michelle has been an integral part of our APP team since 2023. As a Nurse Practitioner, she addresses complex care needs for a vulnerable population and is an excellent communicator within the interdisciplinary team at her center. Meet Dr. Amjad, our new Medical Director who has been a powerful voice for justice and community health within her home of West Virginia. In 2023, Dr. Amjad featured in the documentary film Impossible Town, which follows Dr. Amjad as she takes up her father's mission to help her town exposed to cancer-causing chemicals. "Driven by his call to 'help others' at all costs, Ayne launches an audacious plan to relocate the community. In the ever-evolving landscape of healthcare, long-term care remains one of the most complex and underserved settings, especially for new Advanced Practice Providers (APPs). Recognizing this, Christina D. and Falon B., two clinical education leaders at AlignMed Partners, worked to create EMERGE, a clinical education and mentorship initiative designed to support early-career and new-to-setting providers. In this interview, they share the story behind EMERGE’s inception and the journey toward seeking accreditation through the Advanced Practice Provider Fellowship Accreditation program (APPFA). With a focus on evidence-based practice, mentorship, and tailored educational modules, EMERGE is poised to become the first APPFA accredited APP fellowship in long-term care.
